Practicer Merger - Impact on Online Services
We apologise for the inconvenience. Due to the merger between Rowan Tree Practice and Church Street Practice, online access via the NHS App, Patient Access and other GP linked services will be disrupted. Access to record access and medication requests will cease on 17/05/24.

Please be aware:
- This is not a permanent disruption, but you will have to restore access yourself. We have no automatic way of doing this.
- Urgent prescriptions can still be submitted via our website.
- Records access over this weekend will be down.
From 20/05/2024
We strongly encourage you to install/delete and reinstall the NHS App from Monday 20th of May after 12:30. You can register this way as long as you have access to a smartphone and a form of legitimate photo ID.
This process can be done without having to to wait, visit or contact the practice.
In the event you have lost online access, do not have a smart phone, or for any other reason may need to continue using Patient Access, you will have to attend the practice to fill in a verification form. The practice will then manually process these forms - matching them with existing online users.
You may experience delays with this process, and this process only applies to those with existing online access.
