Birthday Recalls for Long Term Health Conditions
From April 2024, patients with long-term health conditions will now be invited for annual review by month of birth.
These patients include:
- · Asthma
- · COPD
- · Coronary Heart Disease
- · Heart Failure
- · Diabetes
- · Learning Disabilities
- · Hypertension
- · Non-Diabetic Hyperglycaemia
- · Mental Health
- · Dementia
You will be sent 2 invites within the year, after that point your records will show you have declined until your review has been completed.
Please note, patients who have recently had their annual review will still be enrolled for this recall.
We encourage patients to come to any reviews they are invited to.
If your birthday is now not until next year and you feel you are due a review sooner, please do not hesitate book this. Please remember for any reviews requiring a blood test, we will need prior notice to generate the necessary forms. Please inform the receptionist when booking or if you book online you can use the message facility
This only applies to Diabetic/Cholesterol/Non-diabetic Hyperglycaemia/Mental Health/CHD & HF tests. No other test can be added to the form, any other requests must go through the GP as normal.
General blood pressures/Hypertension/Smears and Stroke/TIA invites will still be done ad-hoc.
If your questions have not been answered after reading through, please contact Karen via the message facility on our current website for the moment.
We thank you all for your ongoing support and look forward to helping our patients within our new practice.

Practice Merger News - 15th November
We are very happy to share the news Rowan Tree Practice and Church Street Practice are merging. The two GP Partnerships have decided that to improve the service we both provide that we are a stronger together and we're very happy to start sharing more details and information in the coming months!
LIVI App - 30th September
From 30th September 2022 free GP video consultations via LIVI will no longer be available to our patients. The practice, via the local GP Federation which we are a part of, have decided to end the contract with LIVI due to unsustainable cost increases and our own improved systems for managing remote digital and telephone consultations which mean the service no longer offers sufficient value for money to local patients.
Following the discontinuation of this service we will have access to additional face to face GP appointments in the evenings and weekends at health centres near to the practice as well as offering direct patient bookings for a number of services.
This decision was carefully considered and taken with the knowledge of local patients and their best interests as our top priority.
Local patients who have previously registered with LIVI will be redirected to their own GP practice if they need an appointment or given the option to pay for a private GP consultation with LIVI.
